Mae'n nodi beth sydd ei angen ar eich plentyn i ffynnu, pa gymorth fydd yn helpu, a sut y bydd pawb sy'n gysylltiedig \u2013 gan gynnwys chi, eich teulu, a gweithwyr proffesiynol \u2013 yn cydweithio.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mae'r cynllun yn fwy na geiriau ar bapur. Mae'n rhywbeth sy'n cael ei roi ar waith i gefnogi'ch plentyn mewn bywyd go iawn, bob dydd.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Byddwn yn dechrau trwy gasglu popeth rydyn ni wedi'i drafod yn ystod eich asesiad \u2013 eich barn, dymuniadau eich plentyn, a mewnbwn gan weithwyr proffesiynol eraill sy'n gysylltiedig. Gallai hyn gynnwys yr hyn sy\u2019n gwneud i\u2019ch plentyn deimlo\u2019n ddiogel, beth mae eisiau i ddigwydd, neu pa gefnogaeth mae\u2019n ei ffafrio.
